By completing this form, you will be confirming your taxpayer status. If you choose to opt in to Gift Aid, you will also be agreeing that we can claim Gift Aid on donations you make in the future and have made in the past 4 years.

Gift Aid Declaration

I want to Gift Aid any donations I make in the future or have made in the past four years, to Jami. I am a UK taxpayer and understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all of my donations in that tax year it is my responsibility to pay any difference. Jami will claim 25p on every £1 donated.

Thank you for agreeing to allow each and every one of your gifts made to Jami to be treated as Gift Aid donations.

This enables Jami to reclaim the tax back on any donations you have made to Jami in the last four tax years prior to this year and all future donations until we are notified otherwise. This means each gift you make will be worth 25% more to us, at no extra cost to you. Jami will claim 25p for every £1 you donate.
